Grelle is referred as he in the anime and manga because it's Victorian England and MtF trans is not exactly a thing back then. >> So the other characters just think that she's a gay guy. The problem here is that Grelle refers to herself as female or in a feminine way (like her idea spot where she's Juliet and in a dress, or how she helped Madame Red because she can't give birth either), but these things are also gay stereotypes, so people get mixed up. In the databook thing, there was an "interview" with Grelle where she refers to herself as a lady and the author refers to her as female.
